Financial Aid Withdrawal Confirmation Form

Current term: 2023 fall

If you have initiated a complete withdrawal from any session or the semester during the current term, you must submit this form within 5 days of initiating the withdrawal.

IMPORTANT: Failure to submit this form within 5 days of your withdrawal will result in a return of your Title IV aid based on your percentage of aid earned for the current term.
