Academic Participation Review

A student who has received all failing grades is considered an unofficial withdrawal and federal and institutional rules require ASU to review their financial aid eligibility. A student receiving all failing grades is subject to a Return of Title IV calculation using the midpoint of the term or session. For additional details, see the federal Return to Title IV policy. 


The purpose of this form is for the student to provide proof of academically related activity at ASU beyond the midpoint (50%) of the term or session for any failed course (grade of E). Our evaluation of your documentation will determine if you are eligible to have some or all of your aid reinstated.

Academically Related Activity

The following are some guidelines from the U.S. Department of Education which define academically related activities:

  1. Submitting an academic assignment;
  2. Taking an exam, an interactive tutorial, or computer-assisted instruction;
  3. Participating in an online discussion on academic matters;
  4. An e-mail from the student showing that the student-initiated contact with the instructor to ask a question about the academic subject studied in the course.

Note: Logging into an online class is not sufficient, by itself, to constitute participation in an academically related activity.
