Enrollment History Review

Your enrollment history is under review. The Department of Education has notified ASU that your enrollment history needs to be reviewed due to potential unusual enrollment activity while receiving federal Pell grant funds within the last three years.

Financial Aid and Scholarship Services will request this documentation only if it is needed to complete your financial aid file. This item holds the awarding and disbursement of financial aid if it is listed on your task list on My ASU.

You can view your document status on your My ASU Finances page. Please allow 2-3 business days to show receipt in your My ASU Financing Tasks.

Instructions

Financial Aid and Scholarship Services was unable to determine if you received academic credit at one or more institutions while receiving a federal Pell grant within the last three years. You will need to complete and submit the Enrollment History Review form below to Financial Aid and Scholarship Services.

This policy is not applicable to scholarships or employee tuition benefits. Your Enrollment History Review requirement does not determine your eligibility to register for classes.

Important Info

  • Print your name and ASU ID on the upper right hand corner of all documents submitted.
  • Electronic signatures or confirmation codes are not valid signatures for financial aid purposes.
  • Please note: ASU cannot accept documentation via e-mail. Please submit documentation in-person, by mail or by fax.

Example Reasons for
Submitting an Enrollment History Review

Examples of Support Documentation (not inclusive)

Personal Crisis

1. A detailed explanation of the crisis including the date of occurrence, duration and how it negatively affected your coursework.
2. Support documentation (e.g. physician’s statement, copy of police report, Personal Protection Order, medical documentation).
3. Support documentation regarding what steps you’ve taken to resolve the crisis and successfully move forward (e.g. physician’s statement, academic advisor or third-party agency that assisted you with resolving this crisis).

Death of a Family Member/Roommate/Close Friend

1. A detailed statement including the name of the deceased and his/her relationship to you with an explanation of how this event negatively affected your coursework.
2. Support documentation (e.g. copy of the obituary, notice of the funeral, or death certificate).
3. Support documentation that demonstrates your ability to return to classes and successfully complete your course load (e.g. statement from academic advisor, letter of support from Counseling and Consultation, physician statement).

Personal Illness or Injury
(including medical withdrawal)


1. A detailed explanation of the medical circumstances including the date of occurrence, duration and how it negatively affected your coursework.
2. Support documentation (e.g. physician’s statement, copy of police report, medical documentation).
3. Support documentation regarding what steps you’ve taken to successfully move forward (e.g. statement from a physician, academic advisor or third-party agency that assisted you during your illness or injury).

Illness of a Family Member

1. A detailed explanation of the medical circumstances of the family member, including the name and relationship of the family member to you, the date(s) of occurrence(s), duration and how it negatively affected your coursework.
2. Support documentation (e.g. physician’s statement, medical documentation).
3. Support documentation regarding what steps you’ve taken that will allow you to successfully move forward (e.g. physician’s statement, academic advisor, third-party agency).

Academic Hardship Due to Disability
(including medical withdrawal)

1. A detailed explanation regarding the nature/type of disability and how it’s negatively impacted your academic progress.
2. Support documentation (e.g. physician’s statement, medical documentation, counselor statement) substantiating the nature/type of disability and how it’s impacted your academic progress.
3. Support documentation regarding what steps you’ve taken to successfully move forward (e.g. physician’s statement, Disability Resources for Students, academic advisor or third-party agency that has been and is assisting you with your disability and/or academic progress).

Military Obligations

  1. U.S. military service (in the event of a military deployment)
  2. Reservist called to active duty
  3.  

For either situation please provide a letter from commanding officer.
