TEACH Grant Request
This form is used to request consideration for a Federal TEACH Grant if you were not automatically awarded one once ASU receives your FAFSA and all requested items are complete. Requests do not guarantee awarding of the Federal TEACH Grant. Student Financial Assistance will determine your eligibility for the Federal TEACH Grant based on your citizenship status, academic program and GPA.

Request for Federal Student Loan Due to Parental Nondisclosure
This form is only to be used by students who are dependent for federal financial aid purposes when/if their parents refuse or are unable to complete a FAFSA. Approval of this request will not result in approval for any grants, scholarships, loans (other than Unsubsidized Federal Direct Stafford Loans) or employment that require completion of the FAFSA.
Approval of this form will only provide students with the opportunity to borrow an Unsubsidized Federal Stafford Loan.

Update Parent PLUS Borrower Info
Use this form to update parent PLUS borrower information for Student Financial Assistance when the parent is a former ASU student who attended with a different name or has incorrect date of birth or Social Security Number on file. This form is not used to change a current student’s information.

Statement of Intent and Understanding for Graduate Preparatory Coursework
This form is for students who completed a bachelor's degree and must complete preparatory coursework to qualify for admission into a graduate degree program at Arizona State University. Changes resulting from this review do not guarantee an increase in aid.

Consortium Agreement
Consortium agreements enable Arizona State University (ASU) degree-seeking students to participate in financial aid programs while concurrently attending ASU and another accredited higher education institution. ASU reserves the right to not enter into a consortium agreement if, in the opinion of ASU's SFAO, that consortium agreement would be administratively prohibitive. Consortium agreements assist in receiving additional financial aid, not for the deferment of loans.
