Immunizations

You must use this form to submit proof of one of the following to any ASU health service is required before registration for classes will be permitted:

1) 2 MMR immunizations (must have 2 different dates of injection) At least one of the shots must have been given after 1979 OR

2) Lab test showing immunity to RUBEOLA

This form provides information and consent for women receiving the MMR while pregnant.

This form will be used to record what vaccine was given, when the vaccine was given, the name of the company that made the vaccine, the vaccine’s special lot number, the signature and title of the person who gave the vaccine, and the address where the vaccine was given.
