Veteran's Status Verification

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. You indicated on the FAFSA that you are a veteran, however the VA could not confirm your status. Please complete one of the following actions:
    1. If you are a veteran, submit a copy of your DD214 (member copy 4) "Certificate of Release or Discharge From Active Duty" showing military service other than the Reserves or National Guard (unless you were called to active duty).
    2. If you are not a veteran now but will be by June 30 of the academic year you are applying for, submit a copy of your military ID and separation orders (only if you have never been issued a DD214).
    3. If A or B above do not apply to you, you must correct your FAFSA and include your parent's information.
  2. Submit documentation to Student Financial Assistance at the Tempe campus and include your ASU ID number on each page.

Untaxed Income Verification

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Student Federal Tax Return or Non-Tax Filing Form

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. If you and your spouse (if applicable) are required to file a Federal Income Tax Return (1040, 1040-A or 1040-EZ), please submit a signed copy of your (and your spouse's) tax returns to Student Financial Assistance at the Tempe campus.
  2. If you and your spouse (if applicable) are not required to file a Federal Income Tax Return, please submit the appropriate year's form below to Student Financial Assistance at the Tempe campus.

What Year's Information Do I Submit?

You must submit the appropriate year's tax return (or form below) for the academic year it is being requested.
  • For the 2009-10 academic year, submit a 2008 tax return.
  • For the 2010-11 academic year, submit a 2009 tax return.
  • For the 2011-12 academic year, submit a 2010 tax return.

Social Security Card (Signed Copy)

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. Please submit either:
    • A copy of your signed social security card
    • A letter from Social Security Administration stating your social security number and name
  2. Submit documentation to Student Financial Assistance at the Tempe campus.

Selective Service Registration Proof

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. If you have already registered with Selective Service, please submit one of the following to Student Financial Assistance at the Tempe campus:
    • A copy of your selective service registration card
    • A print out of the selective service registration confirmation page (obtained online at www.sss.gov)
  2. If you have not registered with Selective Service, register at www.sss.gov and then print out and submit your confirmation page to Student Financial Assistance at the Tempe campus.
  3. Please complete the form below if one of the following apply and submit it to one of the addresses on the form:
    • You are a female
    • You were born before 1960
    • You are not required to register

Postseconday Enrollment Verification

Student Financial Assistance will request this form only if it is needed to complete your financial aid file.

Parent's Federal Tax Return or Non-Tax Filing Form

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. If your parents are required to file a Federal Income Tax Return (1040, 1040-A or 1040-EZ), please submit a signed copy of their tax returns to Student Financial Assistance at the Tempe campus. Print your name and ASU ID on the upper right hand corner of all documents submitted.
  2. If your parents are not required to file a Federal Income Tax Return, please submit the appropriate year's form below to Student Financial Assistance at the Tempe campus.

Orphan or Ward of Court Proof

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. You indicated one of the following on Step 2 of your 2009-10 FAFSA that when you were age 13 or older:
    1. you were in foster care,
    2. both parents, or both adoptive parents, are deceased or
    3. you are, or were, a ward of the court until age 18.
  2. If one of the above circumstances is true, please provide Student Financial Assistance at the Tempe campus with one of the following:
    1. a copy of official court documentation that you are in foster care,
    2. copies of both parents', or both adoptive parents', death certificates or
    3. a copy of official court documentation of your status as a ward of the court.
  3. If neither of the above circumstances is true or if one of your parents is still living, you will need to:
    1. provide parental information on Step 4 of your SAR
    2. correct question 55 to read "No" and
    3. you and your parent must sign and resubmit your SAR electronically and
    4. you must notify Student Financial Assistance at the Tempe campus, in writing, that you have updated your FAFSA and included your parent's information.
    You will receive a new SAR in 4 to 6 weeks. If you do not have your original SAR or cannot access it online, contact the Dept. of Education at 1-800-4FEDAID to request a duplicate.

Mid-Year Transfer

Student Financial Assistance will request this form only if it is needed to complete your financial aid file.

Instructions: 
  1. Please download the correct form below and follow the instructions included on it.
  2. Submit the form to Student Financial Assistance at the Tempe campus.

Loan Default Resolution Required

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. Refer to your Student Aid Report (SAR) on the FAFSA website (www.fafsa.ed.gov), call the Department of Education at 1-800-4FEDAID or contact Debt Collection Services at 1-800-621-3115 for instructions on how to resolve your defaulted loan.
  2. Once the defaulted loan is resolved please provide Student Financial Assistance at the Tempe campus with one of the appropriate forms of documentation from the guarantee or collection agency.

Appropriate documentation consists of:

  • Loan paid in full (including consolidation)
  • Satisfactory repayment arrangement
  • Rehabilitation of loan

Household Size Verification

Federal Loan Discharged Due to Disability

FAA Adjustment Required

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. Student Financial Assistance posts this item to your financial aid file if another institution changed your or your parents' FAFSA information due to your unusual circumstances.
  2. Because federal regulations require independent, institutional reviews of unusual circumstances, the changes made by another institution are not automatically considered by ASU.
  3. To have unusual student/parent expenses or a reduction of student/parent income reviewed by ASU, please submit either a Student Review form or Parent Review form.
  4. Attach a copy of the SAR used for the review at your other institution to your ASU review.

Eligible Non-Citizen Proof

  1. Please submit a copy of the front & back of one of the following:
    • I-551 (Alien Registration Receipt Card)
    • I-551C (Conditional Permanent Resident Card)
