Tax Extension

If this item is on your To Do List, Student Financial Assistance has received a tax extension and a request to award you based on estimated tax information. Your aid disbursement is on hold until Student Financial Assistance has received and processed a copy of your completed Federal Income Tax Return.

Instructions: 
  1. Submit a signed copy of yours or your parent's (if applicable) Federal Income Tax Return (1040, 1040-A or 1040-EZ) to Student Financial Assistance at the Tempe campus.
  2. Student Financial Assistance must receive a copy of your tax returns before August 25. Students who do not submit tax returns before this deadline risk losing estimated awards.

Student Dislocated Worker

If you have this item in your To Do list on My ASU, you have reported on the FAFSA that you or your spouse are a dislocated worker or a displaced homemaker.

In general, a person may considered a dislocated worker if he or she:

  • If you feel that you or your spouse meets one of the conditions above for being a dislocated worker or a displaced homemaker, complete a Student Review form and indicate your or your spouse's status.
  • If you or your spouse do not meet the conditions for being a dislocated worker or displaced homemaker, you should also complete a Student Review form and indicate a loss of income.
  • If you or your spouse did not lose or quit your job, please notify Student Financial Assistance in writing so this item can be removed from your To Do list.

Missing Parent Information

If you have this item in your To Do list on My ASU, you have reported on the FAFSA that you are unable to provide parental information.

Instructions: 

You must choose one of the three options below in order for financial aid processing to continue.


Circumstance Required Actions
A. You are unable to provide parental information due to a special circumstance.The following are examples of some special circumstances:
  • Your parents are incarcerated; or
  • You left home due to an abusive family environment; or
  • You don’t know where your parents are and are unable to contact them (and you have not been adopted).
Submit a Dependency Review form to Student Financial Assistance documenting your special circumstance.
B. Your parents do not want to provide their information on the FAFSA or they refuse to contribute to your college expenses, and you would like consideration for a Federal Unsubsidized Stafford Loan. Submit a Request for Federal Student Loan Due to Parental Nondisclosure form to Student Financial Assistance.
C. Neither A or B above apply to you and you would like consideration for federal student aid. You must correct your FAFSA and include your parent's information on it. You must also notify the Student Financial Assistance at the Tempe campus in writing of these changes in order to continue processing.

Unaccompanied or Homeless Youth Verification

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. You indicated one of the following on Step 2 of your FAFSA that at some time on or after July 1, 2008,:
    1. your high school or school district homeless liaison determined that you were an unaccompanied youth who was homeless,
    2. the director of an emergency shelter program funded by the U.S. Dept. of Housing and Urban Development determined that you were an unaccompanied youth who was homeless, or
    3. the director of a runaway or homeless youth basic center or transitional living program determined that you were an unaccompanied youth who was homeless or were self-supporting and at risk of being homeless.
  2. If one of the above circumstances is true, please submit the appropriate year's form below to Student Financial Assistance at the Tempe campus completed by one of the following designated officials:
    1. a McKinney-Vento School District Liaison,
    2. a director or designee of a HUD-funded shelter, or
    3. a director or designee of a RHYA-funded shelter,
  3. If neither of the above circumstances is true or if one of your parents is still living, you will need to:
    1. provide parental information on Step 2 of your SAR
    2. correct questions 58, 59 and 60 to read "No" and
    3. you and your parent must sign and resubmit your SAR electronically.
    You will receive a new SAR in 4 to 6 weeks. If you do not have your original SAR or cannot access it online, contact the Dept. of Education at 1-800-4FEDAID to request a duplicate.

Emancipated Minor or Guardianship Proof

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. You indicated one of the following on the Dependency Status section of your FAFSA:
    1. you are an emancipated minor or
    2. you are in legal guardianship.
  2. If one of the above circumstances is true, please submit one of the following to Student Financial Assistance at the Tempe campus (include your ASU ID number and each page):
    1. a copy of a court's decision that you are an emancipated minor or
    2. a copy of a court's decision that you are in legal guardianship.
  3. If neither of the above circumstances is true or if one of your parents is still living, you will need to:
    1. correct questions 56 and 57 to read "No",
    2. provide parental information on your SAR and
    3. you and your parent must sign and resubmit your SAR electronically and
    4. you must notify Student Financial Assistance at the Tempe campus, in writing, that you have updated your FAFSA and included your parent's information.
    You will receive a new SAR in 4 to 6 weeks. If you do not have your original SAR or cannot access it online, contact the Dept. of Education at 1-800-4FEDAID to request a duplicate.

Citizenship Proof

Student Financial Assistance will request this documentationonly if it is needed to complete your financial aid file.

Instructions: 
  1. Please submit one of the following acceptable forms of U.S. Citizenship proof:
    • A copy of your (the student's) birth certificate showing that you were born in the United States which includes Puerto Rico, Guam, the U.S. Virgin Islands, American Samoa, Swain's Island or the Northern Mariana Islands
    • A copy of Form FS-240 ("Report of Birth Abroad of a Citizen of the United States")
    • A copy of Form FS-545 ("Certificate of Birth-Foreign Service")
    • A copy of Form DS-1350 ("Certificate of Birth")
    • A copy of the Form G-639, (the Freedom of Information Act Form)
    • A United States passport (in the case of nationals who are not citizens, it will be stamped "Non-citizen National")
    • A Certificate of Citizenship, which contains at least the following information:
      • Student's name
      • Certificate number (found in the upper right hand corner)
      • Date of issuance of the certificate
    • A Certificate of Naturalization, which contains at least the following information:
    • Student's name
    • Certificate number (found in the upper right hand corner)
    • Alien Registration Number (A-number)
    • Name of the court where the naturalization occurred
    • Date of naturalization

Important

  • U.S. Citizenship and Immigration Services (USCIS) has confirmed that any of the above documents may be photocopied as long as it is done for lawful purposes.
  • Note that Social Security cards and driver's licenses are not acceptable forms of documentation of citizenship or national status.

Dependency Override

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Student Financial Assistance has been notified that you were eligible for a Dependency Override from another institution. Dependency Override decisions do not remain in effect from one school to another.

Submission of a Dependency Review form is not a guarantee of appeal approval.
