International Student Insurance Waiver

In order to WAIVE OUT of the University-sponsored health insurance plan, international students must complete this waiver form and turn it into the ASU Campus Health Service Insurance office by the 10th DAY of the beginning of each semester.

Exemption Form deadline is January 25, 2010
