Arizona State University participates in a program administered by the U.S. Department of Education called the Quality Assurance Program (QAP). The QAP was designed by the U.S. Department of Education to ensure accuracy in the administration of federal financial aid. As a participant in the QAP, we are required to periodically conduct population sample studies. These mandatory studies help to monitor our efficiency and accuracy while providing data that is vital in planning for future years. You have been randomly selected to participate in our most current study.
Your signature on the FAFSA signifies your agreement to provide verification of your application information to ASU’s Student Financial Assistance office. Your application will be placed on hold until the requested documentation is provided and appropriately completed. Please submit the requested documents as soon as possible, but no later than July 14, 2008. Failure to comply may result in a cancellation of some or all of your aid.

- It is not necessary for you to re-submit tax returns you have already submitted for verification purposes.
