Selective Service Registration Proof
Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.
Instructions:
- If you have already registered with Selective Service, please submit one of the following to Student Financial Assistance at the Tempe campus:
- A copy of your selective service registration card
- A print out of the selective service registration confirmation page (obtained online at www.sss.gov)
- If you have not registered with Selective Service, register at www.sss.gov and then print out and submit your confirmation page to Student Financial Assistance at the Tempe campus.
- Please complete the form below if one of the following apply and submit it to one of the addresses on the form:
- You are a female
- You were born before 1960
- You are not required to register
