Selective Service Registration Proof

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions:
  1. If you have already registered with Selective Service, please submit either:
    • A copy of your selective service registration card
    • A print out of the selective service registration confirmation page (obtained online at www.sss.gov)
  2. If you have not registered with Selective Service, register at www.sss.gov and then print out and submit your confirmation page.
  3. Please complete the form on the right if one of the following apply:
    • You are a female
    • You were born before 1960
    • You are not required to register
  4. Students enrolling at the Tempe or Polytechnic campuses should return this documentation to:
