Orphan or Ward of Court Proof

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions: 
  1. You indicated one of the following on Step 2 of your 2009-10 FAFSA that when you were age 13 or older:
    1. you were in foster care,
    2. both parents, or both adoptive parents, are deceased or
    3. you are, or were, a ward of the court until age 18.
  2. If one of the above circumstances is true, please provide Student Financial Assistance at the Tempe campus with one of the following:
    1. a copy of official court documentation that you are in foster care,
    2. copies of both parents', or both adoptive parents', death certificates or
    3. a copy of official court documentation of your status as a ward of the court.
  3. If neither of the above circumstances is true or if one of your parents is still living, you will need to:
    1. provide parental information on Step 4 of your SAR
    2. correct question 55 to read "No" and
    3. you and your parent must sign and resubmit your SAR electronically and
    4. you must notify Student Financial Assistance at the Tempe campus, in writing, that you have updated your FAFSA and included your parent's information.
    You will receive a new SAR in 4 to 6 weeks. If you do not have your original SAR or cannot access it online, contact the Dept. of Education at 1-800-4FEDAID to request a duplicate.
