Orphan or Ward of Court Proof
Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.
Instructions:
- You indicated one of the following on Step 2 of your 2009-10 FAFSA that when you were age 13 or older:
- you were in foster care,
- both parents, or both adoptive parents, are deceased or
- you are, or were, a ward of the court until age 18.
- If one of the above circumstances is true, please provide Student Financial Assistance at the Tempe campus with one of the following:
- a copy of official court documentation that you are in foster care,
- copies of both parents', or both adoptive parents', death certificates or
- a copy of official court documentation of your status as a ward of the court.
- If neither of the above circumstances is true or if one of your parents is still living, you will need to:
- provide parental information on Step 4 of your SAR
- correct question 55 to read "No" and
- you and your parent must sign and resubmit your SAR electronically and
- you must notify Student Financial Assistance at the Tempe campus, in writing, that you have updated your FAFSA and included your parent's information.
