Orphan or Ward of Court Proof

Student Financial Assistance will request this documentation only if it is needed to complete your financial aid file.

Instructions:
  1. You indicated one of the following on your FAFSA:
    1. You are (or were) a ward of the court until age 18
    2. Both your parents (or adoptive parents) are deceased
  2. If one of the above circumstances is true, please provide our office with one of the following:
    1. A copy of official court documentation of your status as a ward of the court
    2. Copies of both parents' (or both adoptive parents') death certificates
  3. If neither of the above circumstances is true or if one of your parents is still living, you will need to provide parental information (questions 56-89) on your Student Aid Report (SAR). Correct question 53 to read "No". You and your parent must sign and resubmit your SAR electronically or by mail to the address indicated on the report. You will receive a new SAR in 4 to 6 weeks. If you do not have your original SAR, contact the Central Processor at 1-800-4FEDAID to request a duplicate.
  4. Students enrolling at the Tempe or Polytechnic campuses should return this documentation to:
