This document is required in order for ASU to deduct prior year charges from your Federal financial aid. If you do not authorize the deduction from your Federal aid, you may not be allowed important university services.
Instructions:
Please submit this form to the ASU Collections Office by one of the following methods:
- Fax: (480) 965-4587
- Mail: ASU, Collections, PO Box 870303, Tempe AZ 85287-0303
- In Person: Temporary Student Business Services Building (East of the Memorial Union) at the Tempe Campus. Polytechnic, West and Downtown Campus students may deliver this form directly to Cashiering Services on any of these campuses.
Important: Parking charges must be paid directly to Parking and Transit.
